B. Other Updates




  • Select the update that you want to make by ticking the appropriate box.



  • If you selected "Others", specify the update in the space provided.



  • Enter the details of the update in the spaces provided.



C. Correction/Change/Update of Registration Information




  • Select the information that you want to correct, change, or update by ticking the appropriate box.



  • If you selected "Others", specify the information in the space provided.



  • Enter the old and new information in the spaces provided.



D. Closure of Business/Cancellation of Registration




  • Select the reason for closure or cancellation by ticking the appropriate box.



  • If you selected "Others", specify the reason in the space provided.



  • Enter the date of closure or cancellation in the space provided.



E. Change of Civil Status




  • Select your old and new civil status by ticking the appropriate boxes.

Declaration and Signature




After filling out Part I and Part II, you need to sign and date the form at the bottom. You also need to indicate your name, position/title, TIN, and contact number. How to submit BIR Form 1905
