C. Correction/Change/Update of Registration Information




  • Select the information that you want to correct, change, or update by ticking the appropriate box.



  • If you selected "Others", specify the information in the space provided.



  • Enter the old and new information in the spaces provided.



D. Closure of Business/Cancellation of Registration




  • Select the reason for closure or cancellation by ticking the appropriate box.
